Due
Due the last day of the anniversary month of incorporation, every second year on the formation year's parity. Indiana takes the report from 90 days before the month it is due.
First one
First report due two years after incorporation, in the anniversary month.
Fee
$32 on INBiz or $50 on paper (for nonprofit corporations paper is the cheaper option, $20 against $22 online).

Biennial

If it is late

Indiana charges no late fee. Grounds for administrative dissolution only arise once the report is 60 days past due, and you then have 60 days from the Secretary of State's notice to file.
