- Due April 15, shifted to the following Monday when it falls on a weekend. A free 60-day extension to June 15 is available if requested electronically by April 15.

- First due April 15 of the calendar year after formation.
- Fee
- $300 flat, owed even if the LLC owns no property. Paying online by card or PayPal adds a 3% technology fee.

If it is late
A late penalty that starts at $30 and grows the later the report gets, capped at $500, plus interest. Forfeiture is slower - the state certifies its list just after September 30 each year, and only for reports still missing from the year before.
